Be careful to select the right questionnaire for the age of your child or young person

For suspected autism spectrum disorder (ASD)

(1) AQ-10 for children aged 4 to 11 years

(2) AQ-10 for children aged 12 to 15 years

(3) AQ-10 for young people aged 16+ years

Our autism assessment service is available for children and young people from 2 years old, up to their 18th birthday (17 years and 364 days).

All assessments up to 8 years old must be face to face. We are currently able to offer in person assessments at clinic locations in Exeter and Barnstaple.

From 8 years old up to their 18th birthday an assessment may be remote or in person at one of our clinic locations dependent on the suitability for your child or young person; and your preference.

We ask that children under the age of 4 years are seen by a Community Paediatrician before or alongside being referred to our service. This is to ensure that any broader developmental, medical, or genetic considerations are appropriately explored.

For suspected attention deficit hyperactivity disorder (ADHD)

(1) SNAP IV Teacher and Parent rating scale

Our remote ADHD assessment service is available for children and young people aged 8 and above, up to their 18th birthday (17 years and 364 days).

Once you have completed the questionnaire, provide this to your child and young person’s GP.

Your GP will need to complete an online referral form to Held Health.
